COSMETIC SURGERY

Blepharoplasty (Lower Eyelid Surgery) in Utah

Lower blepharoplasty, also called cosmetic lower eyelid surgery or under-eye "bag" removal, removes puffy under-eye "bags." When needed, it can also remove or tighten thin, crepe-paper-like eyelid skin and fill the sunken hollow along the bone beneath the eye with cosmetic fat transfer, so the lower eyelid blends into the cheek in one smooth, youthful curve. Steven Mobley, MD, a double board-certified Facial Plastic Surgeon in Salt Lake City, Utah, calls this approach Contour Sculpting, and he has spent his career perfecting the balance between removing just enough fat, restoring volume with your own fat, and refining the skin with a chemical peel or his Lumenis UltraPulse CO2 laser. Most men and women are back to work in 7–10 days.

IT'S NOT JUST THE "BAGS."

You can't stop seeing them: the puffy "bags" under your eyes, the dark, often sunken hollow beneath them, and the tired face looking back at you. On Zoom calls, you catch yourself studying your own face instead of the person you're meeting with. Women find creams do little and concealer can only hide so much; men are often the first to say their "bags" make them look older and more tired than their younger colleagues. Here's what most people don't realize: when we're young, the lower eyelid and cheek form one soft, smooth curve. With time, that curve breaks into three: a bulge, often a hollow, and then the cheek. It isn't anything you did wrong; it's simply how the face ages. Lower eyelid surgery with Steven Mobley, MD, restores that single smooth curve (Facial Plastic Surgeons call it a "single convexity"), so you look like your younger self: rested and refreshed, never hollow, pulled or overdone. For more than two decades, men and women from Lindon and Provo to Ogden, Idaho, Jackson, Wyoming, Nevada, Texas, California and the East Coast have trusted this double board-certified Facial Plastic Surgeon in Salt Lake City, Utah, with their lower eyelid surgery.

How do I choose the best lower blepharoplasty surgeon in Salt Lake City?

In almost no other area of Facial Plastic Surgery is decision-making as crucial as it is in cosmetic lower eyelid surgery. The skin is among the thinnest on the body, and the right plan matters as much as the surgery itself. Before you choose a surgeon for your lower blepharoplasty, ask yourself these three key questions: 1) Technique, 2) Experience, 3) Leadership in the field.

1. Technique: Will your surgeon look beyond the "bags" to the hollow and the skin?

Typically in our early to mid-40s, the fat pads of the lower eyelid push forward into "bags," and just a few years later the bone beneath the eye (the orbital rim) begins to hollow out. The result is what Steven Mobley, MD, calls a "double convexity": a bulge in the lower eyelid, a subtle hollow below it, and then the cheek: the classic tired, older-looking eyes no one wants. For some patients, removing the "bags," with or without a little skin, is all that is needed. When Dr. Mobley believes it will give you the best final result, he will also suggest softening the hollows with cosmetic fat transfer and improving the quality of the skin with a peel or laser. His Contour Sculpting looks at all three:

  • Step 1, Fat removal: just enough of the bulging fat is removed, never so much that the eye looks hollow.
  • Step 2, Fat replacement: when suggested, a few cc of your own fat, refined into a smooth, gel-like graft, are placed along the orbital rim to soften the sunken hollow. Because it's your own living tissue, the fat that takes is there to stay.
  • Step 3, Treat the crepe-paper-like skin: when suggested, a chemical peel or the Lumenis UltraPulse CO2 laser tightens and refines the delicate lower eyelid skin, so the eyelid and cheek flow together in one smooth curve again.

Together, you and Dr. Mobley agree on your expertly curated plan. Younger patients, often in their 30s and 40s, usually do best through a tiny incision hidden inside the pink of the lower eyelid, with no stitches to remove and often a chemical peel instead of a laser. Slightly older patients usually do best through a fine incision just beneath the lashes, which allows Dr. Mobley to tighten the muscle while removing the puffy "bags" at the same time. His guiding philosophy: never ask too much of any one step. Don't take out too much fat. Don't replace too much fat. Don't take out too much skin. Don't leave too much skin behind. Each step reflects years of refined technique and contributes just its share, and the result looks like you, only more rested.

Stitch technique matters, too, especially when the incision is beneath the lashes. In Dr. Mobley's experience, dissolving stitches, which the body breaks down through its own inflammatory response, tend to keep incisions red much longer, so he takes the time to close with fine removable stitches that come out at about 7 days. At the outer corner, he places the tiny extension of the incision precisely in line with your natural crow's feet. That precision takes extra time, time your eyes deserve, and it makes the final healed incision very difficult to spot, even up close. That is the point of great technique: we simply want your friends to wonder why you look so good!

2. Experience: Does your surgeon have the years of judgment to curate the right plan from the beginning?

The most important decision in lower eyelid surgery is made before surgery begins: the plan. If the right plan isn't curated from the beginning, even beautifully performed surgery can't deliver the strongest result. For more than two decades in Salt Lake City, Steven Mobley, MD, has performed lower eyelid surgery on men and women, on its own and together with upper eyelid surgery, facelifts, neck lifts, brow lifts and cosmetic fat transfer. That experience is what tells him when removing the "bags" is enough and when adding fat or skin refinement will take your result from good to great. Eyelid surgery is one of the fastest-growing parts of his practice, and with 24 years of serving his patients behind him and many more ahead, he operates every week at Millcreek Surgical Suite in Salt Lake City, an AAAASF-accredited surgical suite.

Dr. Mobley is proud to be on staff at St. Mark's Hospital. Readers of The Park Record voted him Park City's Best for 10 years, he has been recognized as a Castle Connolly Top Doctor, and he has been featured on Good Morning America, Inside Edition, People and Cosmopolitan. Many Utahns still remember him as the former ABC4 TV health reporter on Good Things Utah.

3. Leadership in the field: Has your surgeon been invited to national meetings to teach other Facial Plastic Surgeons?

Throughout his career, Steven Mobley, MD, has helped shape how other surgeons approach eyelid surgery: he co-authored a published review on the anatomy of the eyelids for Facial Plastic Surgery Clinics of North America, taught University of Utah surgery residents, and has been invited by the organizers of five national and international meetings, including the annual meeting of the American Academy of Facial Plastic and Reconstructive Surgery and the Global Aesthetics Conference, to teach his techniques for male eyelid surgery. He has delivered more than 7 lectures on eyelid surgery and, over his career, an eye-opening 85-plus lectures in all, and has written more than 50 published works, including peer-reviewed journal articles and chapters in the textbooks surgeons train on.

Dr. Mobley devoted 11 years to the University of Utah School of Medicine as a full-time faculty member, earning tenure as Associate Professor of Surgery and serving as Head of Facial Plastic and Reconstructive Surgery and Residency Program Director. He has been invited to lecture at seven academic medical centers in the United States and Germany, including Kaiser Permanente Oakland Medical Center, the University of Arizona and the University of Marburg.

Dr. Mobley and the Mobley MD practice have given more than $100,000 in charitable surgery and donations, mostly here in Utah: free ear pinning surgery for school-age children (K–12) who are bullied about their ears and whose families could not otherwise afford the operation, Raise the Future's work with Utah foster youth, and Folds of Honor and Tunnel to Towers. As a second-generation surgeon, he was raised to serve his patients and give back to the community they share.

SHOULD I CONSIDER LOWER EYELID (BLEPHAROPLASTY) SURGERY?

Lower eyelid surgery with Steven Mobley, MD, may be right for you if under-eye "bags," a sunken or dark hollow beneath the eyes, or thin, crepe-paper-like lower eyelid skin make you look more tired or older than you feel. Most patients are in their late 30s to 70s, and men are a growing share of Dr. Mobley's lower eyelid patients, many of them tired of seeing their "bags" in Zoom meetings. Because eyelid surgery is elective, your safety comes first: if you take blood thinners, or have significant heart disease or high blood pressure that isn't well controlled, please mention it when you first call so we can talk it through before your consultation.

COMMON REASONS FOR BLEPHAROPLASTY:

  • Puffy "bags" under the eyes
  • Sunken, dark hollows (the tear trough)
  • Under-eye "bags" that show on Zoom calls and in photos
  • Thin, crepe-paper-like (crepey), sun-damaged lower eyelid skin
  • A tired look that sleep, eye creams and concealer can't fix

DR. MOBLEY WILL BUILD A CURATED PLAN FOR YOUR SURGERY

At your consultation in Salt Lake City, Steven Mobley, MD, will take clinical photos and videos to study your eyelids, cheeks and skin, and walk you step by step through what he suggests and why. Together, you'll agree on your expertly curated plan. Many patients pair lower eyelid surgery with upper eyelid surgery, a facelift or a neck lift, so the whole face is refreshed together. If you're one of the many patients who travel from afar to Salt Lake City for Dr. Mobley, combining procedures means one trip, one recovery and one week away from home. Done well, no one will be able to tell you had eyelid surgery. When you get back home, just tell your friends you had a great trip to the mountains, and the fresh mountain air is why you look so rested!

Frequently Asked Questions

The lower eyelid surgery questions men and women ask Steven Mobley, MD, most often in his Salt Lake City, Utah, consultations, answered in plain language.

What does lower blepharoplasty (lower eyelid surgery) involve?

Steven Mobley, MD, removes just enough of the bulging fat that creates under-eye "bags." When he believes it will give you the best final result, he also suggests filling the sunken hollow along the orbital rim with your own fat and smoothing the skin with a chemical peel or the Lumenis UltraPulse CO2 laser, an approach he calls Contour Sculpting. Depending on your anatomy and age, he works through the pink inside the lower eyelid, with no stitches to remove, or through a fine incision just beneath the lashes.

How much does lower blepharoplasty cost in Utah?

Lower blepharoplasty with Steven Mobley, MD, in Salt Lake City, Utah, starts at around $5,500, all-in: Dr. Mobley's fee, the Millcreek Surgical Suite facility fee and your anesthesia. Upper and lower eyelid surgery together start at around $10,000. Adding cosmetic fat transfer or skin refinement, and the length of your surgery (facility and anesthesia fees are time-based), will shape your final price, and every patient leaves the consultation with a clear, written quote. Cosmetic eyelid surgery is not covered by insurance. And at Mobley MD you will never hear "decide today and save"; Dr. Mobley believes same-day sales pressure has no place in healthcare.

What financing options are available?

Mobley MD offers financing through PatientFi, with CareCredit also available, for lower eyelid surgery with Steven Mobley, MD, and our Salt Lake City team is happy to explain how it works.

Will I be put under general anesthesia for lower eyelid surgery?

Yes. Lower eyelid surgery is performed under general anesthesia at Millcreek Surgical Suite in Salt Lake City, an AAAASF-accredited surgical suite. Dr. Mobley's anesthesia team most often uses an LMA, a soft suction cup that rests at the back of the tongue. It is gentler than a breathing tube down the throat and less likely to leave you with a sore throat after your cosmetic eyelid surgery.

When can I return to work after lower eyelid surgery?

Most lower eyelid patients of Steven Mobley, MD, return to work and social plans in 7–10 days, using light makeup or, for men, a tinted sunscreen over any fading bruises. It's perfectly fine to take a few days longer, and if your job is physically active, a few extra days of rest is wise. Plan on about 14 days before strenuous exercise. Out-of-town patients stay in Salt Lake City for about 1 week. Men, take this one seriously: for the first 7–10 days, your result is like freshly poured concrete. Overdo it with heavy lifting, a hard workout or "just one round of golf" too soon, and you can leave a lasting mark on an operation that took years of skill to perfect. Protect it while it sets, and it will look its best for years to come.

How painful is lower blepharoplasty?

Most patients describe the first few days as swollen and a little dry or scratchy rather than painful. Cool compresses, lubricating eye drops and prescription anti-inflammatory eye drops help a great deal. Steven Mobley, MD, wants every patient comfortable and believes in the responsible use of post-op pain medication, with a 3-layer approach (Tylenol, Celebrex and a prescription pain medicine, each in the right amount), explained in his patient video, Understanding Your Pain Medication.

Do I need fat transfer with my lower eyelid surgery?

Not always. Dr. Mobley suggests it when he believes it will give you the best final result. If you have a sunken hollow along the bone beneath the eye, what doctors call the orbital rim, removing the "bags" alone can make that hollow look deeper. Steven Mobley, MD, gently harvests a small amount of your own fat, usually from the thighs or buttocks for women and the abdomen or "love handles" for men, and places it along the orbital rim so the eyelid contour flows smoothly into the cheek.

Can men have lower eyelid surgery and still look natural?

Yes, of course. Steven Mobley, MD, has published on cosmetic surgery for men, including an article on facelift consultation for men in Facial Plastic Surgery Clinics of North America, and organizers of five national and international meetings, including the annual meeting of the American Academy of Facial Plastic and Reconstructive Surgery and the international Global Aesthetics Conference in Miami, have invited him to teach male eyelid surgery, including where to cut and when to fill, to fellow surgeons. For men, he keeps the lower eyelid strong and natural, removes just enough fat and, when suggested, softens the hollow, so the eyes look rested, never overdone. Many of his male patients come in because their "bags" show on video calls, and leave simply looking less tired.

What are the most common risks of lower blepharoplasty? Can I have it after LASIK?

Steven Mobley, MD, will review your risks with you in person in Salt Lake City, and in detail through standard surgical consent forms. The ones patients ask about most are temporary swelling of the eye's surface (chemosis), short-term dryness or irritation, bruising, and small differences between the two sides. In over 24 years, thanks to his "never ask too much of any one step" philosophy, Dr. Mobley has never caused an ectropion (a pulled-down lower eyelid), and he has never caused any loss of vision. Yes, you can have lower eyelid surgery after LASIK; just let us know, along with any history of dry eyes, so Dr. Mobley can better plan your surgery and recovery around them.
